C’est en 2009, dans la loi n°2009-879 du 21 juillet du code de la Santé Publique portant sur la réforme de l’Hôpital et relative aux Patients, à la Santé et aux Territoires (dite loi HPST) qu’a été reconnue la démarche d’éducation thérapeutique. Cette démarche fait appel à différents registres conceptuels et méthodologiques dont celui relatif au diagnostic éducatif et au projet personnalisé. Mais comment faire pour accompagner le patient dans une démarche véritablement personnalisée ? Après un regard critique sur l’existant en la matière, c’est cette méthodologie particulière que propose cet ouvrage : de la collecte des données au projet individualisé et son évaluation en passant par le diagnostic éducatif. Chaque étape de cette méthodologie y est étayée de fondements théoriques ; les principaux écueils rencontrés sont expliqués. Le « comment faire » est décrit pas à pas puis, illustré à partir de situations de patients, en n’oubliant jamais que « chacun, parce qu’il pense, est responsable de la sagesse ou de la folie de sa vie, c’est-à-dire de sa destinée » (Platon). Ce livre s’adresse prioritairement à tous les professionnels de santé (médecins, infirmiers, kinésithérapeutes, pharmaciens, psychomotriciens, diététiciens, orthophonistes, psychologues…) intervenant dans le domaine de l’éducation thérapeutique et plus généralement aux étudiants des métiers de la santé et aux professionnels participant à la construction d’analyses de situation et de projets individualisés ou personnalisés en dehors du domaine même de l’éducation thérapeutique (établissements médico-sociaux, Aide Sociale à l’Enfance…).

Dans l’esprit de tous, la relation entre patient et médecin est simple et naturelle : l’un demande des soins, l’autre les lui prodigue. La réalité est souvent très différente. Soigner est peut-être naturel quand il s’agit de nos proches, mais les professionnels de la santé n’ont souvent ni la formation ni la motivation pour exercer dans les conditions adéquates. Un peu partout, le monde médical reste marqué par une culture peu propice au souci et au respect de l’autre. Certaines attitudes médicales peuvent même être maltraitantes, en particulier à l’égard des femmes. S’appuyant sur des exemples tirés des contextes français et québécois, Marc Zaffran a voulu aller au fond de cette question, qu’il a souvent abordée dans ses différents ouvrages : comment la relation entre patient et médecin peut-elle cesser d’être un rapport de force et devenir, pleinement, une relation de soin - c’est à dire d’entraide, de soutien et de partage.
